Am I right to use Borang 111A of Kaedah-Kaedah Mahkamah 2012 for my appeal to the High Court?

Yes, form 111A.
Do you know the deadline for filing?
Also send a copy to by Registered Mail to bank lawyer on the day you file.

Thanks a lot for your response. The SAR said I have 14 days to send in my notice for appeal at the High Court. The hearing last week was on 11 November 2014. The deadline then would be on  25 November 2014. I will prepare the notice today and hope to file by tomorrow. Many thanks again @Notalawyer.

Are you planning to file the Notice of Appeal at the Magistrate Court counter or the High Court counter?
Do you know the filing fees amount?

Kaedah 55 Aturan 5 says I need to file at the Pejabat Pendaftaran Mahkamah Rendah yang berkenaan and submit a copy to the Pejabat Pendaftaran Mahkamah Tinggi within 14 days from the date of keputusan diumumkan dan menyampaikan sesalinan kepada responden.

Am having a difficulty. Smokeball shows me a different format of form 111A from the one in kaedah mahakamh 2012.

I think smokeball version is a bit outdated.

On the fee, it appears every filing of appeal to High Court will carry a fee of RM200.

The Notis Rayuan doesn't look right.

The judge did not decide on these 2 things.
The judge decided to reject the application set aside the JID
This must be stated correctly in the Notis Rayuan.
You need to correct it.

Make it complete, don't just write (TAJUK) there.
The bottom aldo must state who filed this Notis Rayuan.

Yes, the filing fee will be about RM 200.
